The company is part of Ovations, a family of UnitedHealth Group businesses dedicated to improving the health and well-being of Americans ages 50 and older. Here is a rewarding work from home position, ideal for a skilled RN capable of performing telephonic assessments and home visits to Medicare/Medicaid members in the Houston area. Our members will be in all different stages of healthcare and a self motivated RN is needed to work along side the patients' entire support team (physicians, family members, etc).   • Taking calls from members, vendors and home health providers
  • Obtaining resources and authorizations for members
  • Providing telephonic assessments to determine what services are needed after a hospitalization
  • Educating patients over the phone (regarding care) and making referrals for services
  • Using internal database to document status
  • Attending meetings on updated policies and tools
  • Handling home visits for an assigned caseload
  • Arranging member care (including treatment plans, monitoring their care, etc.)
  • Aiding members with the navigation of the Medicare and Medicaid complex care system of Medicare and Medicaid



Requirements

The chosen RN must possess:
  • A current, unrestricted RN license in their state of residence
  • Clinical experience
  • A home healthcare background
  • Medicare and Medicaid knowledge
Preferred experience: 
  • Bilingual skills in Spanish, Vietnamese or Chinese
  • A stable work history
  • Telephonic nursing experience
  • A long term care history
